1. Waiver forms must be signed and brought to our first session, soccer forms

2. ALL FEES MUST BE PAID INCLUDING REGISTRATION/INSURANCE FEE for 2016 , in order for your child to join our
soccer program

3. ALL KIDS MUST BRING THEIR SOCCER BALLS (new players will receive them at the practice-uniform pick-up

4. ALL SESSIONS ARE SUBJECT TO FACILITY AVAILABILITY, (that a facility is unable to fulfill its rental agreement (facility double-booked, facility closes without notification, etc.) practices will be postphoned and/or rescheduled (please see policy)
